State regulators blame mismanagement for a coronavirus outbreak in a Pulaski assisted living facility.
The Wisconsin Division of Quality Assurance finds administrators at Country Villa failed to adequately monitor residents with coronavirus symptoms and did not follow public health guidelines until it was too late.
Forty-six cases of COVID-19 were tied to Country Villa, with ten residents dying.
Owner Chad Reader is ordered to pay a 76-hundred dollar fine. The facility cannot admit new residents until the violations are corrected.
